I also added the "form-data" dependency directly. The checks were failing when I updated to the new utils... it's likely that we were getting v4 form-data through a peer dependency from axios which got removed recently in a utils PR. I believe form-data is being used as part of the current cloud shell implementation.
I did some simple tests and I don't think this breaks anything but can you double check just in case @alexweininger
