Unzip, deploy inside collectiveaccess/providence/app/plugins directory. There's no need to activate the plugin.
Important : Rename the folder to prepopulatePHP.
At each save, the prepopulatePHP plugin will apply the modifications you've developed in the prepopulate.php file.
