Incorporates #237, #238, #239, #240, and #241 into a single widget that we can incorporate into a page easily.

This is functionally the same thing as our current checkout widget, only it needs to be in React.

Tasks:

Flow from Contact Form to Payment Information to Shipping Information to Billing Information to Thank You forms smoothly
Ensure responsivity at Mobile, Tablet, and Desktop breakpoints, per MaterialUI standards

Acceptance Criteria

Checkout flows smoothly from form to form
Information is saved appropriately
Checkout does not look inappropriate on either side of Mobile, Tablet, and Desktop breakpoints, per MaterialUI standards
