Tua Forma

This plugin just collect and send the fields in the form by email.

Feeling free to define your own fields, styles and etc into the HTML block.

Don't forget to name each field on the form and add the submit button.

Easy WP SMTP

You can use Easy WP SMTP to configure SMTP server in WordPress

Successful and error message

This messages can be modified from > Settings > Tua Forma > [Error message | Successful message] and the style with the CSS classes (tua-forma-successful-message | tua-forma-error-message)
