I'm also able to reproduce what caused the device to be dead. After "flash_all" from agnos-builder, device will reboot and require you to upgrade as the following image. If you choose to upgrade the device, the device will be bricked and flash.comma.ai won't able to flash it. So, we have to choose not to upgrade and use flash.comma.ai to flash the device and it will reset fine.
